Sick (fit) notes
If you’ve been off sick for more than 7 days
To request a sick note:
- phone or visit the surgery from 10am to 6pm
If you’ve been off sick for 7 days or less
If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.

How do i renew my sick note?
Contact the surgery on the day after your fit note runs out. The receptionist will leave a message for the doctor and you will be asked to ring back later that day or the following day to check if it has been completed.
Fit notes can be backdated if you have been seen by a doctor or nurse practitioner here at the surgery or at a hospital. Relevant documentation to the reason for the fit note is needed to be received from the hospital before one can be completed.
It may be necessary for you to be seen by a doctor before the fit note can be issued.
Fit notes are sent via SMS with a link to view it as a PDF. This can then be easily emailed to any relevant party. If you require a paper version please let us know when it is being requested for.
